Hours: 35 hours across 5 days

Salary: £35,102 including OLW + 6% pension contribution

 

Community Connect is a Social Prescribing service, which is an exciting new way of working and is gaining momentum across the country. It gives people the time to explore what matters to them and find non-medical, holistic support to improve quality of life, health, and wellbeing. For professionals, it demonstrates a commitment to partnership working and providing wrap around support for people, keeping them at the heart of everything that we do.  

 

Purpose of Job

Community Connect is a well-established Social Prescribing service and the post-holder will work closely with the Voluntary Sector, Primary Care, and other Local Care Partnership colleagues to ensure a well-embedded, high-quality service. The postholder will be responsible for the development and sustainability of the Community Connect service, aligning Social Prescribing across Bexley and VCS development.

 

Duties and Responsibilities

  • Identify and lead on new opportunities for more effective partnership working to improve health outcomes for individuals and communities, working closely within the Local Care Partnership (LCP), Primary Care, and the voluntary and community sector.
     
  • Manage budgets in accordance with the funding agreement and providing frequent reports on outcomes and learning to commissioners and funding bodies.
     
  • Demonstrate effective leadership and management of the Social Prescribing team, providing a supportive and motivating line-management framework for staff to deliver a high-performance culture.
     
  • Have an active role within the Local Care Partnership, building and nurturing relationships across sectors, and where appropriate, identifying new partnership opportunities which strengthen relationships between Voluntary Sector partners and statutory colleagues.
     
  • Identify opportunities for additional funding and lead in developing and submitting partnership funding bids, securing more funding for Bexley’s Voluntary Sector.
     
  • Provide strategic insight into the Bexley landscape, drawing on data analysis from the Social Prescribing Project and sharing knowledge across partners.
     
  • Liaise with, and support VCS organisations with any challenges which will support more sustainable Social Prescribing Pathways and referrals, including raising relevant challenges within strategic meetings.
     
  • Work closely with other Social Prescribing services to streamline and integrate systems and ensure a consistent and equitable Social Prescribing service within Bexley.
     
  • Be a champion of Social Prescribing, and the voluntary sector, locally, regionally, and nationally.  
  • Undertake any other duties and tasks which may reasonably be required, as discussed with line-manager
  • Adhere to and actively implement BVSC’s policies and procedures such as Equality and Diversity, Health & Safety and Safeguarding.
  • Identify, report, and monitor any safeguarding risks and concerns for Children, Young People, and Adults.
  • Commit to undertaking any training considered necessary to ensure and develop own knowledge and skills to effectively deliver the role

These are the normal duties which are required of the position; however, we do require that all staff be flexible and may be required to perform other duties to ensure the efficient running of services. It is envisaged that this post will be office based 4 days with 1 day at home, but this will be reviewed regularly.
