Role Description

Community Based Frailty Practitioner
Hours: 21 hours per week (0.6fte)
2-year fixed contract
Salary: £ 18,241 (Inc of OLW)

An exciting opportunity has arisen for a part time Community frailty practitioner to join our team to improve the pathways for older people & their carers who present symptoms of frailty. You must hold a relevant professional qualification in health/social community care with a strong background of working with older people. You will be a great communicator with the ability to work independently, IT proficient, have a full driving license and demonstrate a clear understanding of the assessment tools required for this client group. You will provide expert support to identify & assess frail older people who are at risk of further deterioration in health that may result in avoidable hospital admission or unnecessary length of hospital stay. The post holder will be required to work flexibly across our older people’s services in Bexley to ensure service delivery and safety. The post holder will be responsible for working within their own skill set to support carers and individuals at risk of falls to manage their health and wellbeing to live as independently in their own homes for as long as possible.

This role will include responsibility for:
• Carrying out agreed specific assessments including a basic falls risk assessment
• Provide guidance and support to enable individuals to be a self-independent as possible.
• Supporting and working with carers and wider family members.
• Carry out low level falls assessment/screening and provide advice and information on falls prevention, including referral for equipment where necessary.
• Develop, monitor, and review care plans and agreed outcomes in partnership with the person and the carer to evaluate outcomes.
• Providing case management for frail older people who may have multiple long-term conditions and are at risk of deteriorating health.
• Act as the key advocate for carers as and when required
• Identifying and escalating any risks or safeguarding concerns to senior colleagues and actively seek to reduce or eliminate those concerns.
• Working effectively with GP practices, health, social care, housing, and voluntary sector partners.
• Establishing communication networks with other professionals to share good practice.

Our Head Office is based in Erith, your role will be primarily working in the community with older people and their carers residing in the London Borough of Bexley.
