6 month contact initially (pilot project)
Part time role (job share)- either 2, 2.5 or 3 days per week 9:45am to 5:30pm Monday to Friday.
Age UK Bexley is looking to set up a Take Home and Settle Service to facilitate timely discharge from hospital for people over 50, who have been a patient at Darent Valley Hospital and are ready to go home.
The role will involve collecting patients from the hospital by car and take them home with a view to settling them back at home. You will ensure that there are basic food supplies, that heating is on, plugs are put in and patients have everything that they need to hand – any medication and phone numbers etc.
You will also be required to facilitate services to other organizations if continued housework, care or shopping will be required.
You will ideally have experience of working in Health and Social care
You should be a driver and have use of a car with business insurance.
We are aiming to recruit at least 2 people to this role so that we can continue to provide the service when one member of staff is on annual leave.
Salary: £25,000 per annum (full time equivalent); £13.35 per hour
6 month pilot scheme.
The service will operate Monday to Friday 9:45am to 5:30pm
