Role Description

An exciting opportunity has arisen for a part time Community frailty practitioner to join our team to improve the pathways for older people & their carers who present symptoms of frailty. You must hold a relevant professional qualification in health/social community care with a strong background of working with older people.

You will be a great communicator with the ability to work independently, IT proficient, have a full driving license and demonstrate a clear understanding of the assessment tools required for this client group.

You will provide expert support to identify & assess frail older people who are at risk of further deterioration in health that may result in avoidable hospital admission or unnecessary length of hospital stay. The post holder will be required towork flexibly across our older people’s services in Bexley to ensure service delivery and safety.

The post holder will be responsible for working within their own skill set to support carers and individuals at risk of falls to manage their health and wellbeing to live as independently in their own homes for as long as possible.
