You will seek to engage with families and carers, provide emotional and practical support, understand their lived experience and the barriers they face as they support their loved ones who have mental health needs. Key Responsibilities;
· Raise awareness of the needs and experiences of families locally
· Engage with the carers and family’s community network this may include co-delivery of services, collaborating with local agencies and services to offer holistic and tailored interventions
· Provide personalised, strength-based care including comprehensive assessments, 1:1 support delivering brief psychosocial interventions, collaborative care planning and reviews to ensure that familiy goals are identified and achieved
· Identify and refer mental health carers for a carers assessment Under the Care Act 2014
· Facilitate structured group activity
· Drive improvements to local partnership working to improve outcomes for families
· Ensure families get the support they need as early as possible before their problems escalate into crisis.
If you have experience of providing support to families and carers, experience of supporting people with mental health and wellbeing needs with a counselling skills and a non judgemental approach this could be the role for you.
