This new role is part of a programme of Community Mental Health Transformation that aims to fundamentally remodel the way community mental health services operate. It will contribute to breaking down the barriers for patients moving from primary to secondary mental health services and aims to embed a more joined up approach to patient care. You will work as part of a team in close liaison with Oxleas NHS Trust to provide recovery-orientated peer support to people as they transition from being a hospital inpatient to being settled in the community and living independently. The support is likely to include a combination of emotional and practical support to enable the individual to adapt to the initial changes and build effective skills to manage their home & finances, connect with family & friends and pursue social or vocational interests that encourage them to get more involved in their community. Support may include mentoring, coaching, facilitating access to community activities and emotional support.
