Health and Wellbeing Coaching

At BVSC, we believe that good health is about more than managing illness. It is about having the knowledge, confidence, and support to make positive lifestyle changes that improve both your physical and mental wellbeing.

Health & Wellbeing Coaches work alongside individuals to help them identify what matters most to them, set realistic goals, and develop the skills needed to make lasting changes. Whether you are managing a long-term health condition, looking to improve your lifestyle and prevent future health issues, or need support to overcome challenges affecting your wellbeing, our coaching programmes provide personalised, compassionate support tailored to your needs.

The health and wellbeing coaches can help with areas such as healthy eating, physical activity, stress management, sleep, emotional wellbeing, and building healthy habits. They provide encouragement, motivation, and practical tools to help people take control of their health and wellbeing, while also connecting them with other services and resources where appropriate.

We currently offer three Health & Wellbeing Coaching programmes designed to support residents across Bexley.

1). Long Term Conditions Health Coaching
Our Long-Term Condition Health Coaching programme supports people living with multiple long-term conditions to better understand and manage their health, improve their quality of life, and regain confidence in self-managing their condition.

Through one-to-one coaching, individuals are supported to identify personal goals, overcome barriers, and make sustainable lifestyle changes that can positively impact their health and wellbeing.

Eligibility Long-Term Conditions: 

  • Hypertension (High Blood Pressure)  
  • Diabetes
  • Atrial Fibrillation (AF)
  • Chronic Kidney Disease (CKD)

Eligibility Criteria:

  • You may self-refer or be referred by a healthcare professional if you have:
  • Three or more eligible long-term conditions
  • Two confirmed eligible long-term conditions and one suspected eligible condition
  • One confirmed eligible long-term condition and two suspected eligible conditions

We Are Unable to Accept Referrals For:

  • Individuals under 18 years of age
  • People living with moderate or severe frailty
  • Those who are medically optimised
  • Individuals experiencing a mental health crisis


2). VITAL 5 Health Coaching (North Bexley only)

Our VITAL 5 Health Coaching programme focuses on prevention, supporting people to make positive lifestyle changes before serious health conditions develop. This programme is designed to empower individuals to take proactive steps towards better health by focusing on five key areas that significantly influence long-term wellbeing.

VITAL 5 Areas:

  • Smoking
  • Alcohol Consumption
  • Healthy Weight
  • Healthy Mind
  • Healthy Blood Pressure

Working with a coach, you can explore what changes they would like to make, set achievable goals, and build the confidence needed to create healthier habits for the future.

Eligibility Criteria:

To access the VITAL 5 programme, you must:

  • Be aged 18 or over
  • Want to improve at least one of the Vital 5 areas in your life. 
  • Live in North Bexley

We Are Unable to Accept Referrals For:

  • Individuals under 18 years of age
  • Pregnant individuals
  • Those experiencing a severe mental health crisis


3). Community Health & Wellbeing Workers (no referral pathway)
Our Community Health & Wellbeing Workers provide highly localised, person-centred support for residents living in North Bexley.

This programme takes a holistic approach to health and wellbeing, recognising that a person's health can be affected by a range of factors including physical health, mental wellbeing, social connections, housing, finances, and access to services.

Community Health & Wellbeing Workers support residents with a wide variety of issues, including:

  • Physical health and wellbeing
  • Mental and emotional wellbeing
  • Social isolation and loneliness
  • Accessing local services and community support
  • Practical everyday challenges

Unlike traditional services, Community Health & Wellbeing Workers actively engage with residents in their local neighbourhoods through door-to-door outreach, helping to identify and support those who may benefit from additional assistance.

Eligibility:

  • Must live in North Bexley
  • No exclusion criteria apply

Our Community Health & Wellbeing Workers will endeavour to support individuals with any issue within their scope and will help connect residents to specialist services where needed.

Accessing Our Services:

For both the Long-Term Condition Health Coaching and VITAL 5 Health Coaching programmes, referrals can be made by:

  • A GP or other healthcare professional
  • Self-refer here
